‘The engagement and collaboration with the ANP in general practice has been one of the key enablers to the recent successful rollout of the Integrated Cardiovascular Specialist service hub in Wexford. Not only has she provided insightful advice on referral pathways, but has also ensured an open line of communication between the specialist teams and general practice in the management of patients referred to the service with the best possible outcomes achieved.
